Turning Point Community Programs is seeking an Intake Coordinator for our CRP- Rio Linda Program in Rio Linda. GENERAL PURPOSE

Under the administrative supervision of the Program Director or designee, this position is responsible for providing psychosocial, rehabilitation and recovery-oriented services for our members.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. The position is responsible for the development and implementation of person-centered treatment plans that seek to resolve the presenting problem and support the individual in returning to less intensive community settings as soon as possible. Additional support in areas of medication management, housing, counseling and advocacy will be provided as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • Develops and maintains positive working relationship with referring hospitals and outpatient providers.
  • Coordinates intake functions with the Crisis Residential Programs, Intake Team, and all referral sources
  • Conducts interviews in the community and makes recommendations for admission to the program.
  • Completes intake paperwork with clients upon admission to the program.
  • Ensures that necessary documentation is received and processed from referral sources prior to client admission, including medication orders and medication.
  • Introduces the client to the staff on-duty, the assigned service coordinator, and other clients in the facility, provides the client with a tour of the facility and shows the client to a clean room. Confers with program leadership if the client has specific needs and risk factors, for example, medical issues, dietary needs, and language services.
  • Participates in in-service training and workshops.
  • Assesses clients for involuntary hospitalization per WI 5150 and completes necessary forms (after completion of the County certification).
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
  • Conducts psychosocial groups, treatment planning, and discharge planning activities.
  • Insures the safety, health, and well-being of the members.
  • Completes paperwork as assigned in a timely, thorough and efficient manner.
  • Provides support to other staff members as needed.
  • Attends community meetings to represent the Program as assigned.

Knowledge Of:

  • Principles and goals of community mental health.
  • Principles and goals of the “self-help model.”
  • Principles and goals of the “consumer-driven model.”
  • Principles and goals of the “harm reduction model.”
  • Principles, techniques and trends in counseling, psychotherapy, clinical case management, and various treatment modalities.
  • Psychosocial rehabilitation’s treatment and programming.
  • Ability To:
  • Work and communicate with staff, clients, families, community agencies and Professionals.
  • Perform crisis intervention strategies.
  • Work effectively under stress and conflict.
  • Exercise appropriate judgment and decision-making.
  • Be flexible and adaptable in any given situation. 
  • Work as a member of a team.
  • Work under time deadlines and pressures.
  • Work with minimal supervision.
  • Get to multiple locations typically via car.

Schedule: Monday - Friday 8:00 am - 4:30 pm

Compensation: $27.00 - 29.23 per hour

 

 

 
Position Requirements

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from an accredited college or university with a master’s degree in Social Work or related field, and valid registration with the Board of Behavioral Sciences (BBS).

Licenses; Certificates; Special Requirements:

  • Valid registration with the Board of Behavioral Sciences (BBS)
  • California driver’s license & current vehicle insurance/registration
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
