Plan, organize and implement Social Work services by maintaining contact with referring agencies, community resources, and other relevant contacts. Provide appropriate contact, resource, and referral information for all clients. Initiate contact with families providing education on visitation policies and promoting support/involvement. Assist with coordination of external supports. Assist with insurance/funding issues. Assist with medication issues, as necessary. Provide counseling support services, as necessary. Provide Social Work services to patients admitted to the Medical Recovery Unit (MRU) to include, but are not limited to, program management, educational sessions, family consultation, coordination of aftercare services, and coordination of service with external case managers and funding sources. Create, revise and conduct psychoeducational sessions, as necessary. Prepare and maintain required documentation, records, and reports in compliance with all hospital regulations.   Position Requirements MSW, LSW preferred. Minimum two years of experience in a social work setting with mental health or addictions emphasis required. Minimum two years of experience in a medical social work setting preferred.   This position is currently not accepting applications.
