Share Email Opening Title Direct Support Professional -Group Home (Mental Health)  Category Residential Services   Description Position Summary: The primary purpose of this position is to manage the daily operation of the group home program under the supervision of the Residential Supervisor, provide supportive counseling to the residents who are classified as having a severe and persistent mental illness, coordinate with case managers to provide services for individuals, and other activities as outlined in the PCP/Service Plan. Essential Job Functions: Assists individuals with daily activities, personal care, home care and maintenance, and transportation. Participates in implementation of person-centered plan. Directs therapeutic interventions to build socialization, behavior management skills, and daily/community living skills. Directs therapeutic interventions to promote adaptive skill learning and development of leisure time interests/activities. Maintains documentation, progress notes, and other data as needed, including incident and accident reports. Communicates with other staff and supervisor on progress, needs, concerns and changes. Demonstrates knowledge of agency policies and procedures. Attends staff meetings and clinical supervisions, as required. Responds to individual safety needs, including reporting significant changes in behavior and health. Provides high quality customer service, serving as a positive representative for the agency with other professionals, families, and the community at large. Adheres to budget policies concerning credit card usage, as well as computer, phone and electronic usage as outlined by company policies. Provides medication administration and maintains accurate documentation on the Medication Administration Record (MAR). All other duties as required by supervisor to meet the needs of the clients and program.   Knowledge, Skills, and Abilities: Working knowledge, theory, principle and facts about human behavior. Knowledge of behavior management techniques. Knowledge of the characteristics of the population being served. Ability to evaluate and document objectives and progress. Ability to establish and maintain effective working relationships with others. Knowledge of Recovery Model. Ability to work cooperatively, creatively and productively as a member of a team. Ability to establish appropriate boundaries and relationships with staff and individuals served consistent with professional practice and professional ethics. Possess a strong commitment to helping each individual achieve his/her highest level of independence possible. Possess commitment to respect for individual rights while providing a safe living environment for all program residents.       Position Requirements Minimum Qualifications: High School Diploma or equivalent from an institution appropriately accredited by the state Department of Education in which it was obtained required. Valid drivers license and valid automotbile insurance required. Complete within specified time period the trainings as required by agency and service model.
