Wellness Engagement Specialist needed for temporary (3 month), full-time position in Philadelphia.
Provide comprehensive intake services to those in need, ensuring immediate, urgent needs are met. Facilitate referral process to those seeking case management or other services. Provide backup, assessment, support, and brainstorming to direct services staff. Assist with the development and implementation of pilot programs and special assignments. Represent the organization at events, training, and speaking engagements. Support coordinators with team-member interviews, training, and orientations. Ensure high-quality client services across programs.
Provide medical case management for an assigned caseload of up to 15 clients. Provide Intake Coverage and address client and non-client emergencies during each shift. Assign clients to case management programs (as part of treatment team). Coordinate services with Program Partners. Assist with emergency programs; UESF, EFSP, HAFI, etc. Assess and refer as appropriate. Maintain support for people without services, on waitlists, or without direct staff. Provide backup for direct service staff and coordinators as assigned. Counsel those requesting assistance with the Patient Protection and Affordable Care Act. Provide on-site services in the home, community, medical clinics, or hospitals as needed. Collaborate with other service providers and agencies. Serve as an information resource for HIV and other chronic diseases in the community. Participate in the organization's staff and supervisory meetings. Complete written documentation of client interactions within two business days. Perform other duties as required.

BSW/MSW - Expertise in case management and client advocacy. Ability to work with diverse populations and with critical emergencies. Knowledge of or willingness to learn local resources. Sensitive to cultural, economic, gender, and sexual orientation differences. Understanding of medical and psychosocial issues related to HIV disease and other chronic illnesses. |
