Most of the visits are in South County and Jefferson County. This position is temporary for 6-8 weeks. Someone with 32 hours of availability is preferred, but those with at least 20 hours of availability will be considered.   This position requires mileage reimbursed driving to homes around the area to provide services. All PPE is provided. Typical schedules are from 8:30am-5pm Monday through Friday.   Typical tasks include: Assisting the hospice team in understanding social, spiritual, and emotional factors related to the patient’s health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient. Working as a member of a multidisciplinary team Assessing the social, spiritual, and emotional needs/factors in order to estimate the patient’s and involved caregiver’s capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness. Helping the patient, family, and involved caregiver to understand, accept and follow medical recommendations and provide services planned to assist in achieving the optimum social, spiritual, and health adjustment within their capacity and level of health literacy. Preparing the patient to deal with the changes and the family to support the patient including education on Advanced Directives. Utilizing all available resources, such as family, hospice, and community agencies, to assist the patient and family to live better within the limitations of the illness. Providing discharge planning related to change of level-of-care or community placement.
