SUMMARY: Under general supervision, the Community Health Worker serves as a point of contact between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

 

PRIMARY DUTIES AND RESPONSIBILITIES:

The job description is subject to change at any time by the employer. Essential duties and responsibilities may include, but are not limited to, the following:

 

  • Promotes and supports access to health programs, such as COVID-19 testing and vaccination, COVID-19 follow up care and resources, chronic disease screening and management services, and health insurance enrollment, as appropriate.
  • Facilitates and supports access to social services that aid users in navigating education, employment and workforce development resources, food insecurity and supplemental nutrition programs, and mental health services.
  • Meets regularly with the Community Health Worker Coordinator to assist in developing equity plans and the implementation of the plans.
  • Supports ongoing public health and pandemic response through contact tracing, case investigations, and non-clinical aspects of testing and vaccination efforts.
  • Assists in qualitative data collection such as focus groups and key informant interviews.
  • Collects data in accordance with grant requirements to assess the pilot program; may complete evaluation forms or use instruments developed by the evaluator.
  • Supports the relationship between the Teton County staff and the general public by demonstrating courteous and cooperative behavior when interacting with visitors and County staff; maintains confidentiality of work-related issues and County information; performs other duties as required or assigned.

 

 

 
Position Requirements

MINIMUM QUALIFICATIONS:

 

Education and Experience:

  • High school diploma or GED equivalent AND two (2) years of experience in a social services or medical environment; OR an equivalent combination of education, training, and experience.

 

Required Licenses and Certifications:

  • Must possess a valid Wyoming/Idaho license and maintain an acceptable driving record; if not a Wyoming resident, it is expected that you hold a current Driver's License from another state and will obtain a Wyoming Driver's license within a reasonable amount of time (no more than 90 days).
  • Must be a certified Community Health Worker or obtain certification within six (6) months of hire.
  • Must be a certified Community or Medical Interpreter or obtain certification within six (6) months of hire.

 

Required Knowledge of:

  • Applicable local, State, and Federal laws, codes, regulations, and ordinances.
  • Proper grammar, spelling, and punctuation.
  • Project management practices and methods.
  • Social media platforms and communications.
  • General office policies, procedures, practices, and equipment.
  • Community health programs and resources.
  • Covid-19 pandemic response and team roles.
  • Records maintenance and retention practices and policies.

 

Required Skills in:

  • Gathering data.
  • Assists in planning and implementing public outreach programs.
  • Preparing clear and concise reports and communications.
  • Maintaining accurate records and documentation.
  • Working effectively independently and as part of a team.
  • Preparing and delivering public presentations.
  • Interacting tactfully and professionally with County staff, outside agencies, and the public.
  • Communicating effectively, both verbally and in writing.
  • Establishing and maintaining effective working relationships.

 

Physical Demands/Work Environment:

  • Work is performed in a standard office environment. May be exposed to Covid-19 and other respiratory illnesses.

Hiring Range $23.28 - $26.34 per hr
