Designs, implements and monitors individual program plans for clients. Assesses client needs; investigate their eligibility for services. Counsels and aids individuals and families requiring agency assistance. Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs. Collects all pertinent documents to prepare clients for applying SSD/SSDI. Participates in outreach initiative to engage clients in need of this service. Networks with other providers to generate community resources for participants. May provide supportive counseling in individual and group settings. May transport consumers to appointments, etc. All other assigned tasks by either SOARS case manager or the Program Director.   Position Requirements MINIMUM QUALIFICATIONS: Bachelor’s degree in human services field, rehabilitation, nursing, or a related discipline.  Bilingual Spanish speaking a OR relevant professional credential such as CPRP, CRC, CADC + 2 years related exp., OR a combination of one or more years of college + related work or life experience together equaling four years; LICENSES/CERTIFICATIONS NEEDED:  Valid driver’s license is needed. ~cb   Exempt/Non-Exempt Exempt   Weekly Work Hours 35   Proposed Salary $20.00 per hour   Schedule Monday- Friday, 8am-4pm; some evening hours required  This position is currently not accepting applications.
