Turning Point Community Programs is seeking an On Call Clinician for our CRP Henrietta Program in Sacramento. Turning Point Community Programs (TPCP) provides integrated, cost-effective mental health services, employment and housing for adults, children and their families that promote recovery, independence and self-sufficiency. GENERAL PURPOSE:

Responsible for the application and implementation of developed person-centered treatment plans that seek to resolve the current presenting difficulty and support the individual in stepping down to less intensive community settings as quickly as possible.  Additional support in the areas of medication management, resource connection (e.g., housing resources, AOD, PCP, OP, etc.), counseling and advocacy will be provided as needed.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. The position employee is responsible for assisting and advocating for our members in all areas of treatment and helps them in addressing barriers that prevent them from achieving chosen meaningful roles as well as supporting opportunities for clients to make informed decisions regarding their mental health treatment in order to enhance quality of life roles.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • In the absence of adequate clinical staffing, the on-call clinician completes clinical tasks as assigned.
  • Provides rapid clinical assessment and treatment planning for individuals utilizing program services.
  • Develops a clinical formulation and provisional diagnosis for individuals receiving services.
  • Provides comprehensive risk assessments and initiates 5150 evaluations when deemed appropriate.
  • Enters timely clinical documentation that meets standards determined by the County and reflects person-centered care.
  • Provides appropriate referrals and linkages for effective care coordination.
  • Conducts effective care coordination with existing providers to ensure continuity of care post-discharge.
  • Coordinates comprehensive discharge planning that seeks to reduce risk factors, promote client resiliency and improve quality of life. Completes related discharge summary and other discharge documentation.
  • Provides “on-the-spot” support that is both helpful to the clients and consistent with the philosophy of the program.
  • Collaborates with the program’s interdisciplinary team members regarding client care coordination.
  • Facilitates clinical groups and individual counseling when appropriate and as assigned.
  • Supports all facets of overall program operation, including assistance with client medication coordination, data entry, transportation and report preparation.

Schedule: Varies based on the needs of the program

Compensation: $27.00 - 28.65 per hour

Position Requirements

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from an accredited college or university with a master’s degree in Social Work, Psychology, or related field and two (2) years varied experience as a provider of mental health services; valid registration with the Board of Behavioral Sciences (BBS)

Licenses; Certificates; Special Requirements:

  • Valid registration with the Board of Behavioral Sciences (BBS)
  • California driver’s license & current vehicle insurance/registration if driving; and,
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.
