1. REQUIRED LICENSES OR CERTICIATIONS: Registered Nursing License in the State of Wisconsin
JOB DUTIES AND RESPONSIBILITIES:
1. Evaluates all hospital referrals, coordinates admission process and completes all admission documentation.
2. Assess and evaluate information provided by the D.O.N.
3. Monitor care related informarion and lab results that influence resident acuity, treatment, diagnosis, and classification
4. Assure resident care plans are completed and updated timely in compliance with Medicare/Medicaid rules and regulations.
5. Coordinate the assessment schedule with the inter-disciplinary team to ensure timeliness of assessments and maximize reimbursements for all Medicare PPS assesments.
6. Maintains compliance with requirements in compliance program/plan.
