Turning Point Community Programs is seeking an Assessment Clinician (Formerly known as: Personal Service Coordinator III) for our Urgent Care Program in Sacramento. GENERAL PURPOSE
Responsible for assisting community members with an urgent mental health need. The program intends to avert psychiatric emergency room visits and involuntary hospitalizations. The goal of MHUCC is to foster recovery for individuals and families through the promotion of hope and wellness.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our members in crisis with an urgent mental health need.
ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

- Provides rapid clinical assessment for individuals and families utilizing mental health urgent care services.
- Develops a clinical formulation and provisional diagnosis for individuals needing services.
- Provides comprehensive risk assessments and initiates 5150 evaluations when deemed appropriate.
- Enters timely clinical documentation that meets standards determined by the County of Sacramento and reflects person-centered care.
- Provides appropriate referrals and linkages for effective care coordination.
- Collaborates with on-site clinician provided by the County of Sacramento, in order to access community resources.
- Conducts effective care coordination with existing providers to ensure continuity of care post-discharge.
- Develops and implements a protocol for following up with individuals and families as necessary.
- Provides “on-the-spot” support that is both helpful to the clients and consistent with the philosophy of the program.
- Collaborates with the clinic’s interdisciplinary team members regarding client care coordination
Compensation: $28.00 - $29.71, plus swing shift differential pay.
Schedule: Tuesday - Saturday 4:00 pm - 12:30 am
