In this key position you will use a variety of skills- Health Coach collaborate with our Behavioral Health team to deliver professional, team based integrative care, and nursing supportin a clinical setting Patient Advocate- Bethe liaison between family, providers, hospitals and agencies Connector- Identify resources for patients with difficulty accessing care Social Determinants of Health- Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs ofpatientsand theirfamilies Qualified Candidates possess- Compassion, tenacity, and an eagerness to go the extra mile to improve the health of our community Current RN licensed in Iowa andIllinois, BSN preferred. Minimum of 1 year experience with direct behavior health, clinical and/or managed care experience preferred.
