Description

JOB SUMMARY:

  • Greet all clients to the Connection Center in person and via phone
  • Interact with and assist individuals seeking support for healthy behaviors; including emotional and behavioral health, physical/nutrition health, and spiritual health
  • Assist with completion of intake forms and registering clients into the Electronic Medical Record and scheduling appointments for Navigation support
  • Provide information and referral resources as appropriate via phone and in person for clients and community partners
  • Maintain resource documents in lobby
  • Assist in maintaining social media platforms - including Facebook, Instagram, Twitter
  • Maintain Partner and Team Directories
  • Provide administrative support to Community Health staff, teams and committees
  • Take minutes during all required meetings.
  • Process invoices and track departmental expenses
  • Prepare mailings for events and other solicitations
  • Assist with meeting coordination including room reservations, updating calendars and working with appropriate hospital departments to arrange catering, room set up and equipment needs
  • Assist with all healthy365 events which may include tracking RSVPs, registration, processing monetary income and reporting
  • Assist in managing/maintaining special community programs; ie: transportation/food delivery
  • Assist in creating and maintaining new processes and procedures for healthy365 programming

QUALIFICATIONS:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Excellent people skills, including donor and volunteer relationships.
  • Strong written and oral communication skills.
  • Exceptional organizational skills, ability to multi-task, listen actively and creatively problem solve.
  • Computer proficient in Microsoft Office, donor database knowledge a plus.
  • Results-oriented, self-motivated, team player.
  • Ability to maintain strict confidentiality.
  • Knowledge of local community resources and treatment referral options
  • Comfort navigating social media platforms
  • Maintaining calm demeanor in high stress situations

EXPECTED BEHAVIORS:

  • Possess a sense of responsibility, initiative, honesty, resourcefulness, patience and congeniality.
  • Follow directions accurately and cooperatively.

Position Requirements

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • High School Diploma or Equivalent.
  • Preferred Community Health Worker or related certification.
  • Required 1 - 3 Years administrative support.
