Position Senior Case Manager  
Full-Time/Part-Time Full-Time  
Location Ronkonkoma  
Days/Hours Tuesday-Saturday; 9am-5pm

The Senior Case Manager is responsible for providing linkages with community resources, daily living skills training, counseling and support, documentation, and compliance with program requirements. The Senior Case Manager is responsible for assisting the Program Director with needed staff training, as well as being an ongoing support and resource for other staff.

Duties and Responsibilities:

  • Provide case management services (goal planning; assistance in forming linkages and/or referrals to clinical services, day programs, vocational services and opportunities; medication monitoring; community integration; entitlements etc.)
  • Meet with clients in their supportive housing setting, as required.
  • Complete appropriate documentation in a timely manner, including:
  • Site Inspections
  • Progress Notes
  • Support Plans/Reviews
  • Functional Assessments
  • Recertifications
  • Provide housing support services to help clients maintain independence in their supportive housing setting; collaborate with the Property Management Dept as needed.
  • Provide crisis intervention and support the Program Director in the event of any on-call needs.
  • Assist the Program Director with maintaining program occupancy rates, including the collection of supporting documentation per LIHTC requirements, and support throughout the admission process.
  • Act as a liaison with other service providers and community agencies.
  • Provide training to new staff and refresher trainings to existing staff, as needed.
  • Attend weekly supervision and monthly staff meetings.
  • Remain current with all Agency required compliance documents and trainings.
  • Other tasks as assigned by supervisor.





Position Requirements

Bachelors Degree preferred in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
