Description

General Summary: A nonexempt position responsible for performing a variety of duties including but not limited to assisting patients in navigation within healthcare system, providing appropriate referrals for continuing care, facilitating health and disease patient education, supporting patient self-management of disease and coordinating continuity of patient care with patients and families throughout the healthcare delivery system.

Essential Job Responsibilities:

  1. Supports patient self-management of disease and behavior modification interventions.
  2. Coordinates continuity of patient care with external healthcare organizations and facilities.
  3. Coordinates continuity of patient care with patients and families following hospital and/or skilled nursing facility admission, discharge, and ER visits.
  4. Manages rising-risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting.
  5. With input from patients, families, and the care team, create outcome goals and an overall care plan.
  6. Conducts comprehensive, preventive screenings for patients and/or assists all support staff in daily patient interactions as needed.
  7. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
  8. Facilitates patient medication management based upon standing orders and protocols.
  9. Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
  10. Other duties as assigned.

Performance Requirements:

Knowledge:

  1. Knowledge of health care field and medical office protocols/procedures.
  2. Knowledge of specific assisting tasks related to particular medical practice.
  3. Knowledge of information that must be conveyed to patients and families.

Skills:

  1. Skill in performing operations on Excel database.
  2. Skill in tact and diplomacy in interpersonal interactions.
  3. Skill in understanding patient education needs by effectively sharing information with patients and families.

Abilities:

  1. Ability to learn and retain information regarding patient care procedures.
  2. Ability to project a pleasant and professional image.
  3. Ability to plan, prioritize, and complete delegated tasks.
  4. Ability to demonstrate compassion and caring in dealing with others.
 
Position Requirements

Required Education: High school diploma or general equivalency diploma (GED). Medical Assistants must have graduated from a medical assisting program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or by the Accrediting Bureau of Health Education Schools (ABHES). Licensed Practical Nurse (LPN) must have active license within the state of Indiana.

Experience: Three to five years of recent experience working in a medical practice as a certified medical assistant or LPN. Must be proficient in computer skills, including Microsoft Office (specifically Word and Excel), self-disciplined, energetic and passionate. Should be highly organized and be well-developed in oral and written communication skills. Must demonstrate sound judgment, decision-making and problem-solving skills.

Required Certificate/License: Certification to work in a clinical role providing direct patient care required. LPN must possess active current Indiana licensure. BLS certification thru the American Heart Association (AHA) and TB certification thru the American Lung Association (ALA) preferred.
