Health & Wellness Nurse

Health and Wellness Nurse needed for the Western Region of North Carolina , working with a great Residential I/DD program.


  • Coordinates and oversees initial and ongoing health assessment activities.
  • Promotes best practice/evidence based health promotion tools and resources for direct/clinical staff to utilize to support individuals in residential programs;
  • Visits select Group Homes on a regular, unannounced basis and monitor compliance;
  • Monitors physical & mental health status indicators, symptoms and medications in collaboration with direct care and clinical staff.
  • Reviews and monitors assessments for health risk (HRST), mental health status, smart technology needs;
  • Provides Cclinical consultative input to direct care and clinical staff, and quality management team; Cconsultsation for service/care coordination and person-centered planning;
  • Provides Ttraining to direct care/clinical staff for individualized health needs and medical instructions;
  • Provides Cconsultation for individual, PCP, and/or care staff;
  • Evaluatesion of living settings for health & safety-enhancing, cost-saving smart technology;
  • Provides regular reports on work activity and outcomes generated;
  • Tracks and monitors personal expenses with support from finance support staff;
  • Participate in Policy & Procedure development as related to supporting best possible health for persons receiving services.
  • Review High Frequency Medication Errors for West Region
  • Travel in perspective region is required with occasional overnight stay.


Position Requirements

Persons in this position must have:

  • Verified status as a Registered Nurse in the state of North Carolina (or with reciprocating state) with a current, unrestricted nursing license.
  • One year of documented experience with populations served preferred.
  • Demonstrate high moral and ethical character, and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry;
  • Current, unrestricted, state-appropriate driver’s license;
  • Maintain valid insurance for personal vehicles utilized for work.
  • Complete all assigned trainings as required by agency and supervisor.
