Medical Social Worker needed for full-time, temp to hire position in Philadelphia.
Responsible for discharge planning and contributing to the patient's treatment plan through an understanding of social, psychological, and/or environmental needs related to the impact of illness and need for hospitalization.
Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting. Collaborates and cooperates with other members of the interdisciplinary health care team to ensure continuity of care. Identifies and attends to the psychological needs of patients and families. Provides counseling to patients, families, and groups. Responsible for documentation in the medical record, statistics, and reports as required. Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable. Maintains relationships with community agencies and keeps up-to-date on resources. Uses effective interpersonal skills in all interactions. Enhances own professional development and maintains formal and informal continuing education for self-improvement.
Other duties as assigned to support the unit, department, entity, and health system organization.

MSW and 1-year health care experience. Excellent communication and writing skills, ability to operate with limited direction; ability to multi-task; high-level attention to detail. Extensive knowledge of medical terminology, human anatomy and physiology, and clinical disease processes. Knowledge and understanding of human development and behavior, and ability to make psychosocial assessments. |
