Social Work Consultant Position, Skill Creations, Inc. Durham and Triangle Facilities
Seeking a Social Worker on a consultative basis to assist with providing enhanced social functioning for 27 adult individuals with physical and/or intellectual disabilities and /or medical care needs, residing in a 24 hour group home.
This position will complete Initial Social Work evaluations to include compiled social history of new client admissions. Social Worker will also prepare the discharge summary which includes a 30/60/90 day follow up for discharged clients. This position will represent the family/guardian concerns at initial and annual person centered planning conferences, interim core meetings, quarterly psychotherapeutic reviews, and etc. Case management will be provided through ongoing assessment and interactions with clients/families via regular observations and interactions with clients on site. The social worker will make contacts with guardians as needed to keep them updated on the client’s habilitation process and needs. Advocating/interpreting client’s rights and assisting with needed consents will also be provided through this position. Direct services will be provided through assistance with community contacts as needed with the MCO’s, DSS, Court proceedings, medical information, Insurance needs, funeral/pre-needs for final arrangement planning, and etc.
Starting Salary: $50–$60 per hour (negotiable depending on relevant experience and/ or BSW, MSW degree or higher). Hours at Sanford: up to 8 per MONTH; Hours at Durham (Triangle): Up to 6 hours per MONTH.

Applicant must have a Bachelor in Social Work, Master’s in Social Work or higher. A valid driver’s license and driving record that meets the agency’s insurability standards is essential. Must have own computer and basic computer skills are needed for e-mail, electronic training modules and recording data.
