Description

JOB SUMMARY: Responsible for engaging clients by providing assistance with cases, transportation, and babysitting services. Provide case management services within the scope of agency practices.

ESSENTIAL FUNCTIONS

  • Transport clients to and from program.
  • Provide babysitting services for clients while in treatment
  • Entering billing into Anasazi EMR for transportation and childcare clients.
  • Act as group assistant as needed
  • Assist with meal/snack prep
  • Meet program needs as schedule allows.
  • Develop and implement a program for young children
  • Plan and implement activities to meet the physical, emotional, intellectual and social needs of the children in the program
  • Provide adequate equipment and activities
  • Ensure equipment and the facility are clean, well maintained and safe at all times
  • Develop culturally appropriate programs and activities
  • Be familiar with emergency procedures
  • Supervise children in the day care
  • Ensure children are supervised at all times
  • Provide various experiences and activities for children including songs, games and story telling
  • Provide opportunities for sleep and rest time
  • Build children’s esteem
  • Establish routines and provide positive guidance
  • Provide a safe and secure environment for children to feel comfortable
  • Clearly and effectively communicate in a manner that children understand
     
  • Transport clients and their children in agency vehicle.
  • Service Coordination and Documentation
  • Identifies and accesses community resources to support clients
  • Consults with other service providers and documents their participation in the recovery process.
  • Maintains accurate case records in accordance with agency standards, accreditation and funding source requirements within 24 hours
  • Completes weekly progress notes and documents all direct and indirect activity on behalf of clients.

OTHER DUTIES

  • Participate in the Quality Improvement process and specific quality initiatives for the program.
  • Weekend and evening coverage related to program need
  • Utilize supervision and training as opportunities for enhancing professional competence.
  • Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards.
  • Other related duties as assigned.
 
Position Requirements

Qualifications:

        • High School Diploma
        • Valid New Jersey Driver’s License
        • Excellent driving record
        • Familiarity with the Mercer County area
        • Good interpersonal skills
        • Medical clearance and fingerprinting required
