Case Management Department Manager, Philadelphia

Case Management Department Manager needed for full-time, temporary position in Philadelphia.

The Case Management Department Manager will: manage the activities of the Case Management department of the health system (including supervision of the RN and Social Work Case Managers), utilize professional assessment and treatment skills; coordinate care pre-admission through discharge; promote utilization of available resources to achieve quality cost-effective outcomes; communicate and serve as a liaison to the patient, the clinical team, the family and the payer or payer's representative; maintain technical skills and collaborate with physicians and other interdisciplinary team members to develop a comprehensive/goal-directed plan of care. Ensure weekly utilization review activities are performed by Case Managers and attend weekly case conferences. Acts as a Case Manager for an assigned group of patients as needed.

Position Requirements

MSW with at least 2-3 years of relevant case management experience required. Experience in utilization review and supervision of staff preferred. Must be knowledgeable and demonstrate competency in current principles and practices clinically specific to critical care or medical-surgical, rehabilitative and gerontological nursing, and case management required including discharge planning which is clinically specific to medical/surgical and rehabilitative areas. Must have excellent written and verbal communication skills and must be able to interact and communicate successfully with varying members of the interdisciplinary team.
