Description

ProMedica Senior Care, formerly HCR ManorCare, is recruiting for a Post-Acute Care Hospitalist.  The position is responsible to provide administrative, management and medical services required to ensure the adequacy and appropriateness of the care rendered to assigned patients and residents in assigned locations during scheduled shifts.  The role provides advice, service and consultation in person, by telephone and through written reports and statements in accordance with currently accepted professional standards and applicable federal, state and local laws and regulations.

  • Oversees the ongoing implementation of locations Resident Care Policies and provides guidance to Non-Physician Provider(s) and nursing personnel on their execution related to assigned patients and residents.
  • Consults with and advises the Administrator and Director of Nursing on the medically related aspects of patient and resident care and center operations, employee health community relations and staff development which may impact the care of assigned patients and residents.
  • Visits patients and residents as required to fulfill the staff Physician responsibilities and ensures through appropriate supervision of the physician extender(s) the fulfillment of these responsibilities in which the Non-Physician Provider(s) shares such responsibilities.
  • Collaborates with practice team to establishes policies and procedures to provide for essential routine and emergency physician coverage and services including coverage provision 24 hours per day.
  • Participates as member of the location’s QAPI Committee, Infection Control Committee, Pharmacy Committee and Safety Committee when assigned or deemed appropriate.
  • Arranges for a qualified physician, acceptable to the practice group and the assigned location, to perform the staff physician responsibilities outlined, during any absence, vacation or other periods when unavailable.
  • Provides in-service education for professional nursing staff and Non-Physician Provider (s) as requested or as need indicates to enhance the effectiveness and appropriateness of the care rendered to assigned residents of the assigned locations.
  • Reports to the Administrator or Director of Nursing upon the conclusion of each visit, any issues which may impact the care of assigned locations’ patients and residents.
  • Cooperates fully with the assigned locations in an effort to respond to and resolve any care complaints filed by or with respect to an assigned patient or resident or any investigation instigated by the assigned center in a timely and effective manner. Cooperates fully with insurance company providing protection to the assigned locations with investigations.
  • Promptly notifies the assigned locations of any inquiries, claims and investigations and cooperates fully with the direction of the assigned location with respect to these; monitors infectious control issues and outbreak policies.
  • Provides appropriate and timely documentation related to patient and resident care to meet ProMedica Senior Care policy, federal, state and local laws, regulations and guidelines. Provides appropriate and timely billing documentation for reimbursement under the federal Medicare Program, state Medicaid Program and commercial and managed care insurance plans.
  • Ensures, through appropriate supervision of Non-Physician Provider(s), all assigned patient and resident care and billing documentation is appropriate, medically necessary and timely.
  • Promotes participation in the patients and residents assessment and care planning, monitoring charges in patients and resident's medical status and provides consultation or treatment when called upon by the assigned centers.

 

 
Educational Requirements Licensed to practice Medicine in the State, board certification in primary care, internal medicine or geriatrics preferred.  
Position Requirements

At least 2 years of medical practice experience providing care and services to the geriatric population.
