This position will primarily serve individuals who are positive for HIV/AIDS and struggling with poverty and homelessness. LMSWs in this position will be eligible to have their clinical supervision fees reimbursed. The therapist will: Meet with clients in person and/or virtually for individual and group therapy sessions. A majority of clients will be short-term therapy arrangements. Conduct home visits. Plan and conduct psycho-educational group sessions regularly. Evaluate client needs, create care plans, and develop individual interventions, while setting person-centered goals. Along with Care Coordinators, perform regular wellness checks by phone to inquire about behavioral health status and medication adherence.   Position Requirements   This position is currently not accepting applications.
