Description

JOB PURPOSE:

Provide comprehensive case management to Veterans and their families who are homeless or at risk of homelessness. To provide quality of life and maintain the highest possible ability to function within the community. Provides direct client services, and compiles related documentation. Work involves orienting all eligible participants to the program and providing housing search, placement and supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing.

 
Position Requirements

ESSENTIAL JOB RESPONSIBILITIES:

  • Develop and manage comprehensive outreach/working relationship with stakeholders, including other community members, especially landlords. Be active in and network at monthly Veteran community groups. Provide presentations at various community groups within the greater metropolitan and surrounding area. Promote the success and reputation of the Supportive Services for Veteran Families program. Design, implement, and evaluate an annual landlord recruitment and retention plan. Maintain awareness of changes in market and community processes that can impact services; bring forward prospective recommendations to deal with expected changes.
  • Provide mediation and advocacy with landlords on the client’s behalf to develop a workable plan to obtain and or maintain housing. Assist participants in locating and securing housing of their choice. Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors). Serve as an ongoing liaison between property managers and participants as well as between participants and neighbors. Apply knowledge of residential lease agreements to educate clients of their tenant rights and responsibilities. Assess housing barriers of individuals and families experiencing homelessness to determine housing and service needs.
  • Advocate for and facilitate Veterans’ access to community resources; housing assistance, utility assistance, and relevant community services and benefits. Build database of community housing resources. Provide referrals to community resources as needed.
  • Assure that initial intakes meeting with Veteran occurs; completion of service plan with Veteran including housing and other related needs occur; coordination and monitoring of services occur; tracking of Veteran activities in relation to service plan; conduct housing inspections for clients seeking rental and/or deposit assistance; maintain comprehensive client caseload to include daily detailed case notes, reviewing and submission of Temporary Financial Assistance (TFA) payments by meeting regularly with Site Supervisor to staff case load and provide on-going program evaluation. This may include conducting Peer Audits of files and maintain on-going communication, as needed, with the clients.
  • Demonstrate exceptional customer service, in everything you do, by placing the child, family, Veteran or client first to support our mission to “Empower people to build better lives for themselves, their families and their communities.”

 

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor’s Degree in Social Work, Sociology, Psychology or related field.

EXPERIENCE: 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment. Experience with homeless, low income, veterans & their families a strong plus.

LICENSES: LMSW, LBSW, LMFT preferred. Drivers License with clear record required.

VEHICLE: Must have daily use of a vehicle without prior notice. Up to 30% travel within county and surrounding areas.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Preferential hiring consideration given to Veterans or military spouses.
