Case Manager (Family Empowerment Center) needed for full-time, temp to hire position in SW Philadelphia.
Provide direct service to families as specified in the DHS Performance Standards. Participate in the initial joint visit with DHS for families as needed. Represent the agency and families at JJS and truancy court hearings as needed. Attend and participate in school reviews and meetings as needed. Maintain case records for each family as specified in the DHS Performance Standards. Maintain contact and coordinate the participants for each family to ensure that all are aware of the goals of the family service plan and each party's responsibilities in the achievement of these goals. Conduct regular visits to the families' homes and community agencies servicing the families. Transport families to necessary collateral visits as specified by FES supervisor. Communicate with referring agency as needed.
Communicate with schools, medical offices, behavioral health services, and other agencies for each family for continuity of care and data gathering. Ensure completion of all documentation promptly.
Responsible to have a thorough knowledge of and assuming a major portion of the responsibility for the evaluation and case planning process to ensure that the program is meeting the ongoing physical and treatment needs of each family in the program. Coordinate discharge planning from the program with the family and FES supervisor. Relate to participants in the program by being readily accessible to them.
