Seeking to hire a Full Time Residential Program Manager for the Residence program

Job summary

  • Performs case assignments
  • Ensures the databases and med sheets are completed monthly by the appropriate staff
  • Facilitates clinical staff meetings and provides direction/guidance to staff Ensures medical appointments are made ongoing with consumers
  • Assists the Director with development of clinical and administrative outcomes
  • Works with director to monitor department budget and tracks income in relation to budget goals
  • Ensures that programming reflects acceptable standards for care
  • Participates in all areas of discharge planning, including arranging follow-up visits with a mental health professional, communicating with insurance companies, and providing help with housing transportation, vocational guidance, legal assistance, support groups, medical care and referral to other services or agencies
  • Oversees the coordination of benefits: Tax rebates, Home Energy Assistance Program (HEAP) and Rental Assistance Program (RAP) as well as medical benefits
  • Acts as an advocate for the client in ensuring continued quality of care and access to social services. Advocacy will include, but not be limited to, exploring possible funding sources for care, such as mental health coverage on insurance policies, Medicaid or Medicare
  • Provides  and ensures clinical oversight for all team clients by ensuring that all cases are appropriately assessed on an ongoing basis and developing comprehensive treatment plans through the multi-disciplinary team
  • Ensures and assist in structuring a therapeutic milieu
  • Reviews referral packets to assess clinical appropriateness for the program
  • Ensures the completion of psychiatric evaluations by the psychiatrist or APN
  • Maintains communication with Greystone Park Psychiatric Hospital, Bergen Regional Medical Center or other area hospital to ensure intake process is completed thoroughly and within timeframes consistent with policy and procedures
  • Assigns case manager to visiting client and ensures that the comprehensive assessment is completed within established timeframes
  • Ensures documentation of clinical portion of the comprehensive assessment
  • Participates in special projects, trainings and committees throughout the department and agency at large
  • Ensures completion of monthly reports/surveys as assigned by the department director
  • Ensures the department is compliant with all applicable standards; including DMHAS, Medicaid, JCAHO, Life Safety Codes
  • Ensures that all documentation is compliant with all applicable standards; including DMHAS, Medicaid as well as the agencies internal documentation policies
  • Provides emergency coverage through mandating and other means but in the event all options are exhausted is expected to work in group homes until coverage is found
  • Available to assist staff whenever clinically appropriate
  • Works with Coordinators to provide coverage in group homes

Qualifications

  • Master's Degree in a Mental Health or Behavioral Health discipline from an accredited college or university
  • Two (2) years’ experience in providing treatment services to adult psychiatric population is desirable
  • Valid driver's license insurable by agency's automobile insurance
