Purpose

To supervise the day-to-day nursing activities of the facility in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility, and as may be required by the Assistant Director and Director of Nursing to ensure that the highest degree of quality care is maintained at all times.

Essential Functions

  • Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
  • Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained.
  • Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
  • Inform family members and inform responsible party/sponsor of changes in residents condition.
  • Call funeral homes when requested by the family.
  • Participate in developing, planning, conducting, and scheduling in-service training classes that provide instructions on "how to do the job," and ensure a well-educated nursing service department.
  • Implement and maintain an effective orientation program that orients the new employee to your shift, its policies and procedures, and to his/her job position and duties.
  • Ensure that all personnel performing tasks that involve potential exposure to blood/body fluids participate in appropriate in-service training programs prior to performing such tasks.
  • Ensure that an adequate supply of personal protective equipment are on hand and are readily available to personnel who perform procedures that involve exposure to blood or body fluids.
  • Ensure that assigned work areas (i.e., nurses' stations, medicine preparation rooms, etc.) are maintained in a clean and sanitary manner.
  • Ensure that unit's resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
  • Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained on your unit/shift at all times to meet the needs of the residents.
  • Participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident as necessary.
  • Encourage the resident to participate in the development and review of his/her care plan.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
  • Ensure that all personnel involved in providing care to the resident are aware of the resident’s care plan.
  • Ensure that the nursing notes reflect that the care plan is being followed when administering nursing care or treatment.
  • Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room.
  • Ensure that all nursing service personnel are knowledgeable of the residents' responsibilities and rights including the right to refuse treatment.
  • Review complaints and grievances made by the resident and make a written/oral report to the Assistant Director /Director of Nursing indicating what action(s) were taken to resolve the complaint or grievance. Follow the facility's established procedures.
  • Periodically review the resident's written discharge plan. Participate in the updating of the resident's written discharge plan as required.
  • Interpret the department's policies and procedures to personnel, residents, visitors, and government agencies as required.
  • Attend departmental and staff meetings as directed or called.
  • Attend and participate in in-service educational classes and on-the-job training programs, etc., as scheduled or directed.
  • Admit, transfer, and discharge residents as required.
  • Complete accident/incident reports as necessary.
  • Participate in employee performance evaluations, determining your shift's staffing requirements, and making recommendations to the Assistant Director /Director of Nursing concerning employee dismissals, transfers, etc.
  • Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. Provide leadership to nursing personnel.
  • Ensure that all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program.
  • Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Review complaints and grievances made or filed by your assigned personnel.
  • Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc.; ensure that rooms are ready for new admissions.
  • Greet newly admitted residents upon admission. Escort them to their rooms as necessary.
  • Make rounds with physicians as necessary.
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures.
  • Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.
  • Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status.
  • Notify the resident's attending physician and next-of-kin when there is a change in the resident's condition.
  • Obtain sputum, urine and other specimens for lab tests as ordered.
  • Take and record TPRs, blood pressures, etc., as necessary.
  • Provide direct nursing care as necessary.
  • Assist the Assistant Director /Director of Nursing in monitoring seriously ill residents as necessary.
  • Authorize the use of restraints when necessary and in accordance with established policies and procedures.
  • Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Meet with residents, and/or family members, as necessary. Report problem areas to the Assistant Director /Director of Nursing
  • Assist in arranging transportation for discharged residents as necessary.
  • Performs other related assignments as required.

Work Schedule

  • As assigned, including some week-ends, evenings and Holidays.

Working Conditions

  • Works in well-lighted/ventilated areas.
  • Most of the work is performed indoors
  • Is subject to frequent interruptions.
  • The noise level in the work environment is usually moderate.
  • Is involved with residents, personnel, visitors, government agencies, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, etc.
  • Communicates with the medical staff, nursing staff, and other department supervisors.
  • Works beyond normal duty hours, on weekends, and in other positions temporarily, when necessary.
  • May be exposed to bodily fluids, infectious waste, diseases, conditions, etc., including the AIDS and hepatitis B viruses.

Physical Requirements

  • While performing the duties of this job, the employee is occasionally required to stand; walk; use hands to finger, handle, or feel; implements or office equipment; reach with hands and arms; stoop, kneel, crouch, or crawl and talk or hear.
  • Must be able to speak and write the English language in an understandable manner.
  • Collect information through observation, listening, touching, and smelling.
  • Must have auditory ability sufficient to monitor and assess
  • Job requires regular sitting, standing, walking, stooping, bending, kneeling, crouching, reaching, pushing, pulling, twisting, writing, and carrying charts, food, trays, etc.
  • Must be able to move intermittently throughout the work day.
  • Specific vision abilities required by this job include close, distance and color vision and the ability to adjust focus.
  • Employee may be required to move chairs, beds, and medical equipment, move patients in bed (with or without assistance).
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents, personnel, and support agencies.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • May be required to lift, push, pull, and move equipment, supplies, etc., in excess of fifty (50) pounds.
  • Must be able to assist in the evacuation of residents.

Safety, Sanitation and Infection Controls

  • Assure the assigned work areas are maintained in a clean, safe and sanitary manner.
  • Assure that established infection control and universal precautions practices are observed.
  • Report all hazardous conditions or equipment to your supervisor.
  • Follow established safety policies and procedures.
  • Dispose of refuse daily in accordance with established sanitation procedures.
  • Keep work/assignment areas free of hazardous objects, unnecessary equipment, supplies, etc.
  • Report any accident/incident to your supervisor no matter how minor.
