Seeking to hire a Full – Time Case Manager  for the Homeward Bound Program

Job summary

  • Effectively communicates orally and in writing; completes all required documentation accurately and within the established timeframes.
  • Knowledgeable of community resources and has the ability to seek out additional resources for the program.
  • Develops and maintains professional relationships with collateral agencies
  • Accepts individual differences and works with the culturally diverse populations present among the program’s target population.
  • Plans, coordinates and facilitates activities appropriate for the program’s population.
  • Appropriately assesses client needs and functioning
  • Responsible and accountable for providing supportive and clinical services to young adults while addressing basic housing needs
  • Completes comprehensive assessments/evaluation of client at intake/admission, during on-going treatment/services delivery, and at discharge/transfer
  • Formulates psychosocial treatment plans, with relevant goals, behaviorally-measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Coordinates and follows through with clinical care recommendations made by the program clinician
  • Meets with youth six days per week to ensure that concrete needs are met and to provide in-home services and supportive interventions, including case management, linkage to community resources, provides inter-agency coordination of treatment and responds to clients for crisis intervention. 
  • Develops and implements a life skills education program
  • Serves as advocate for the rights and needs of the youth
  • Undertakes any activities that facilitate the client’s sustainability in the community
  • Documents assessments, treatment plans, progress reports, and discharge summaries in the clinical record, with all entries authenticated and time/dated  
  • Maintains accurate and timely records for each case including all related telephone contacts, meetings and face-to-face contacts on and off site
  • Plans, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum 
  • Monitors compliance with contract standards and submits all necessary documentation
  • Assesses discharge criteria
  • Meets with service providers to ensure that necessary services are in place prior to client discharge
  • Functions as a member within an interdisciplinary team in the assessment/evaluation of clients, and to articulate verbally and in written documentation the integration of client history and assessment information. Actively participates in program and team meetings, provides support for team colleagues, and engages in program-related improvement processes and activities
  • Provides leadership within the team, engages in inter-organizational care coordination, and documents service outcomes
  • Participates in weekly team case conferences and trainings
  • Recognizes and addresses gaps in service and actively collaborate for resolution
  • Participates in quality assurance activities
 
Position Requirements

Qualifications

  • Bachelor’s degree in a behavioral health discipline form an accredited college or university
  • At least one (1) year experience working with children and adolescent populations (added) along with a minimum of one (1) year work experience in related field preferred
  • Must have strong clinical skill set to complete comprehensive assessments and coordinate and follow through with clinical care recommendations made by the program clinician
  • Must have ability to respond to clients for crisis intervention
