We are looking for a compassionate and experienced Hospice Social Worker to join our interdisciplinary team. As a Hospice Social Worker, you will assist patients and families to cope with psychosocial challenges in achieving their health care goals. Hospice Social Worker will utilize a holistic approach to assess patient and family/caregivers needs relative to coordinating appropriate care, develop, implement, and update interdisciplinary treatment plans, consult with and educate patient and family, and facilitate referrals to other healthcare professionals and programs.
