Position Summary:

The Individuals Support Specialist focuses on increasing the individual’s ability to live as independently as possible, managing the illness, and reestablishing his or her community roles related to the following life domains: emotional, social, safety, housing, medical and health, educational, vocational, and legal.


Essential Job Functions:

  • Provides coaching, mentoring, and consultation to the individual served to promote recovery, self- advocacy, and self-direction.
  • Promotes wellness management strategies, which includes delivering manualized interventions
  • Models recovery values, attitudes, beliefs, and personal action to encourage wellness and resilience.
  • Supports and empowers the individual to exercise his or her legal rights within the community.
  • Provides support services interventions and case coordination as needed.
  • Participates in the provision of and provides oversight to ensure structured rehabilitative interventions to individuals served such as Individual Housing & Tenancy Sustaining Services, Money Management and Entitlements, Activities of Daily Living, Personal Health, Wellness and Recovery, Promote Community Integration.
  • Prepares and submits documentation accurately and on time.
  • Participates in peer review.
  • Participates in weekly supervision
  • Completes required training within prescribed timeframes.
  • Other duties as assigned.


Position Requirements

Minimum Qualifications:

  • A minimum of a high school diploma or equivalency required from a USDOE or CHEA accredited institution.
  • Have the knowledge, skills, and abilities to successfully provide services to adults with mental illness and substance use disorders
  • Computer skills to complete daily notes in electronic health record
  • Excellent oral and written communication skills.
  • Ability to handle multiple responsibilities and assign priorities.
  • Must have current, unrestricted, state-appropriate driver’s license.
  • Must have current, unrestricted vehicle insurance and provide own transportation for use in fulfilling job requirements.
