Behavioral Health Intake/Referral Coordinator

REACH is a crisis prevention, intervention and stabilization system to support individuals with intellectual and/or developmental disabilities, as well as a mental health condition or challenging behavior that is negatively affecting their quality of life. REACH provides consultation, prevention, development of crisis education & prevention plans, community based mobile support and crisis therapeutic home services.

Position Summary:

This position is primarily responsible for facilitating the referral process and evaluating the eligibility determination of individuals served by the REACH program. Primary duties include receiving and processing referrals, requesting, evaluating and maintaining individual records, assigning eligible cases to REACH program staff, providing program overview to referral sources, establishing rapport with individuals, families, and systems within catchment area, and maintenance of individual services database.

Essential Job Functions:

  • Implements REACH supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs and those persons over 18 years of age who are not formally diagnosed with an I/DD Diagnosis
  • Collaborates with Clinical Director to evaluate diagnostic criteria and determine client eligibility in accordance with program standards.
  • Creates and maintains spreadsheets and other data tools for data collection, reporting purposes, and case assignments.
  • Facilitates opening/closure processes of REACH clients in electronic health record, data reporting system, internal tracking system, and client notification.
  • Maintains linkages and relationships with community partners.
  • Participates in daily triage meetings and other required meetings with Team Members, REACH Director, CTH staff and other applicable parties.
  • Collaborates with Case Managers, Clinical Home Providers, CSB representatives and/or applicable crisis responder (Emergency Services, Mobile Crisis Teams, Law Enforcement, Hospital Emergency Departments, Walk-in Crisis, etc.).
  • Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services within established timeframes and according to set protocols.
  • Assigns all new clients to appropriate REACH Clinical Coordinator and/or REACH Team.
  • Provide resources and systems linkages to individuals who do not meet criteria
  • Other duties as assigned/required.
Position Requirements

Minimum Qualifications:

  • Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP) – Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and no less than one year of fulltime relevant experience.
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs.
  • Knowledge of the MH and I/DD systems of care.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
