Job Summary

The Transportation Coordinator has the primary responsibility of reviewing and coordinating non-emergent medical transportation requests for patients/clients. The Transportation coordinator will also furnish transportation for patients/clients to medical appointments and treatment facilities as needed. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

Essential Job Functions

  1. Schedules all Yellowhawk medical transports either through internal transportation providers or other brokerage services.
  2. Assists the Transporation Manager with ensuring that the department delivers quality patient/client care and services in a responsible, respectable, caring manner.
  3. Obtains demographic information, medical appointment location, time, date and length of appointment.
  4. Establishes relationships with outside vendors (ie: Oregon Health Plan / Medicaid GOHBI Non-Emergent Transportation) to improve access of timely transportation to and from medical appointments.
  5. Educates patients regarding alternate transportation resources.
  6. Works with appropriate Yellowhawk staff to ensure that medical referrals are in place prior to scheduling transportation to appointment.
  7. Transport Yellowhawk patients/clients to and from appointments and treatment as needed.
  8. Ensure compliance with the Yellowhawk Transportation Policy.
  9. Assist patients/clients in and out of wheelchairs, cars, etc.
  10. Coordinator will not make exceptions nor deviate from the scheduled transports without specific permission from the Transportation Manager.
  11. Transport patients/clients 400 miles or more as assigned.
  12. Document each transport on the approved transportation form.
  13. Clean all Public Health GSA vehicles and ensure maintenance and upkeep.
  14. Maintain confidentiality in accordance with the Yellowhawk Tribal Health Center Policy Manual, HIPAA and the Privacy Act.
  15. Other duties as assigned.

 

 
Position Requirements

Minimum Qualifications

  1. High school diploma or Equivalent.
  2. One (1) year of successful administrative experience required.
  3. CPR/First Aid certification or the ability to obtain within ninety (90) days of employment.
  4. Demonstrated experience with Microsoft Office programs and/or relevant software to meet the needs of the Transportation Program.
  5. Demonstrated high level organizational, problem-solving and planning skills.
  6. Demonstrated ability to manage multiple tasks at once and high level of organizational and planning skills.
  7. Must be trained on how to operate all wheelchair accessible vehicles within thirty (30) days of employment.
  8. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  9. Must submit to and pass a reference and criminal background check.
  10. Must submit to and pass a pre-employment drug and alcohol screening.
  11. Proof of COVID-19 Vaccination.

Preferred Qualifications

  1. Associate’s degree in Business or related field preferred.
  2. Patient transporting experience preferred.
  3. Strongly prefer experience effectively coordinating calendars in a healthcare environment.
  4. Experience working in a Tribal community or Tribal organization.
