Email Opening Title Health Educator - Updated  Description Job Summary Develops and provides health education programs and content that help individuals, families, and the CTUIR community maximize and maintain healthy lifestyles. Oversees the community garden sites and ensures they are maintained. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements and all applicable policies. Essential Job Functions Collects and analyzes data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Supervises Health Promotion Specialist, Food Systems and Garden Specialist, Community Gardener, and other assigned staff; includes responsibility for hiring, training and development, annual evaluations, monitoring performance, and all other aspects of supervision. Leads the Pamanaknúwit Team (health promotion) towards meeting its quarterly objectives in the Annual Work Plan and actively participates as a team member. Oversees and collaborates with physical activity programs. Acts as the primary contact for all health education/activities and the Good Health and Wellness in Indian Country grant. Represents Yellowhawk with a respected and viable presence in the community. Completes all necessary programmatic reporting for GHWIC. Offers nutritional, lifestyle and behavioral modification in collaboration with medical providers. Receives referrals from providers for patients with special health education needs. Areas to be addressed will include but not be limited to, diabetes, cardiovascular disease, weight control, geriatrics, maternal and child health and other disease states. Serves as a member of the health care team and specialty areas relating to nutrition and health education. Provides consultation and training to administrators, health professionals and food service personnel. Collaborates with P/R Manager to develop social media campaign regarding Public Health programs. Provides information and education to the tribal community on topics that may include basic nutrition, food preparation, food selection and meeting of nutritional requirements through one-on-one counseling and group activities. Provides necessary input and documentation for Public Health Accreditation and serve as a member of the Public Health Accreditation Team. Identifies areas for improvement through performance management tracking and conducts quality improvement studies. Other duties as assigned.   Position Requirements Minimum Qualifications Bachelor of Science Degree in Health Related Field. 3 years of successful experience in health education programming/development. Must be able to successfully obtain Lifestyle Coach Certification within 90 days of hire date. Certification necessary to be an active participant in the health promotion and disease specific teams. Demonstrated ability to communicate effectively, both in writing and oral, in any situation, with peers, staff, management, local, state and federal agencies and the general public, on various topics including but not limited to policies, procedures and general operations. Demonstrated experience with computer software, e.g., Microsoft Office, Electronic Health Records, RPMS. Demonstrated excellent interpersonal skills. Demonstrated high level organizational and planning skills. Demonstrated ability to manage multiple tasks at once. Must possess a valid driver’s license to perform job duties that involve driving GSA vehicles both on and off the Umatilla Indian Reservation and maintain insurability to operate a GSA vehicle. Must submit to and pass a reference and criminal background check and a pre-employment drug and alcohol screening. Proof of COVID-19 vaccination. Preferred Qualifications Master’s Degree in Health Related Field. Experience and knowledge in a tribal healthcare system regulated by accreditation standards/guidelines, i.e., Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) and Public Health guidelines. Experience and/or knowledge of history, culture and health needs of Native American communities preferred.
