Early Intervention Service Coordinator/Educator

Rewarding position providing quality case management and developmental education services to children from birth to age three with developmental delays and disabilities. Early Intervention Service Coordinators are responsible for coordinating services, ensuring the success of Individualized Family Service Plan (IFSP) teams, assisting with transition planning and serving as a resource for caregivers.

Provide services to clients

  • Assists families in the development of an Individualized Family Service Plan based on family concerns and priorities.
  • Assists families in developing outcomes and identifying appropriate Early Intervention services.
  • Helps with the coordination of Early Intervention support systems and services.
  • Monitors service delivery and family satisfaction with Early Intervention services.
  • Assists with referrals to appropriate childcare providers and other community resources as needed.
  • Assists families in transitioning from the Early Intervention program to other support systems and services.
  • Assesses developmental levels, with approved evaluation instruments and checklists.
  • Performs individual needs assessments and assists with the development of child and family outcomes.
  • Provides direct education services, including developmental information and activities to children and families in home and/or community settings, based on outcomes identified in the IFSP plan.
  • Provides supervision to interns, as needed.

Document Services

  • Submits case notes for home visits/ face to face visits according to schedule.
  • Documents all contacts on behalf of the child/family in individual client log.
  • Documents services and progress toward outcomes in accordance with Medicaid and State Part C regulations.
  • Maintains accurate and organized client files that clearly documents child progress.
  • Becomes familiar with the Early Intervention Practice manual and adhere to the rules and regulations put forth by the Virginia Part C office.

Collaborate with program staff, the community, families, and other therapists

  • Facilitates IFSP meetings and ensures that all pertinent information is conferred to all team members, and that appropriate actions are being taken.
  • Coordinates and attends transition conferences for Part B school services.
  • Collaborates with community agencies to provide disability and early childhood development training, as needed.
  • Attends departmental staff meetings, as well as, individual meetings with the Program Supervisor.
  • Participate in mandatory Early Intervention trainings, as required.

Position Requirements
  • Bachelor’s degree from accredited four-year college/university in Early Childhood Education, Child Development, Special Education, Psychology, Social Work, Human Services or related field required.
  • Experience working with young children in their natural environments is preferred.
  • Knowledge of child development and recommended practices in service provision for children with or at risk for developmental or behavioral/emotional disabilities.
  • Ability to work independently with minimal supervision.
  • Outstanding oral and written communication skills including clarity and articulation of thought, appropriate grammar, style, form and punctuation.
  • Infant-Toddler Connection of Virginia certification as a Service Coordinator within two weeks of hire.
  • Completion of required Kaleidoscope training within 12 months of hire.
