Description

JOB SUMMARY: This position will work closely with the pre-intake clinician II and the bilingual CHW to assist the CMHC expansion. This position will go out into the community and work with all clients on the waiting list for outpatient services within CFS and current clients at CFS outpatient. In addition, this position will coordinate with Level II IXP navigator during working hours only to assist with appropriate referrals internally at CCT and externally as well.  This position will often refer to PerformCare for appropriate referrals for care.

ESSENTIAL FUNCTIONS:

Regularly meet with pre-intake clinician to discuss clients on the waiting list

Make referrals as necessary

Regular communication with MRSS staff, ACCESS staff

Community visits to assess need and de-escalate crisis

Respond to school, home, community setting during a behavioral/mental health crisis of a client on our waiting list or current client (not for SI/HI)

Phone/Telehealth sessions may take place as appropriate

Maintain contact with family until intake takes place or next session if existing client

If determination is made for external referral, follow up is required

Documentation of all calls and sessions in Anasazi in a timely manner

Maintain close working relationships with FGP Program Director and FAC Program Supervisor to learn of all changes to programming, etc

Case Management: including referrals in the community or warm hand off to CCT staff

Advocacy

Work closely with both the CHW Bilingual Navigator and the Crisis response Pre-Intake Clinician

Possible supervision of graduate students fulfilling field work requirement toward Master’s Degree in Social Work

Work closely with Level II IXP navigator on 911 calls involving mental/behavioral health of children

Maintain working relationships with PerformCare clinical staff and stay up to date on all changes to making referrals

Make appropriate referrals/warm hand offs to PerformCare and other programs within Mercer County

Attend Mercer County community meetings as requested

OTHER DUTIES : as assigned by Clinical Director

KNOWLEDGE/SKILLS/ABILITIES:  computer literacy; clinical skills

SUPERVISORY REQUIREMENTS:  no

 
Position Requirements

MINIMUM QUALIFICATIONS:     LSW, LAC, LPC,or LAC and at least 3 years of clinical experience, bilingual

Preferred

LICENSES/CERTIFICATIONS NEEDED:  Clinical license required (LCSW, LPC, LAC, LSW).
