Description

JOB SUMMARY: This position will work closely with the Pre-Intake Clinician II and the Crisis Response Clinician under CMHC to provide translation and assistance in all areas. 

ESSENTIAL FUNCTIONS:

Assist Pre-Intake Clinician with translation for the all of the following tasks:

Determine appropriateness of referral for outpatient

Make referrals as necessary

Regular communication with ACCESS and MRSS staff

Verification of insurance

Phone or in person Pre-Intake

Maintain contact with family until intake takes place

Warm hand off for referral, if internal

If determination is made for external referral, follow up is required

Documentation of all calls

Case Management: including referrals in the community or warm hand off to CHW navigator and/or care coordinator

Advocacy

Assist Care Coordinator with the following tasks as required:

Complete NOMS and assessments

Advocacy

Referrals

Communication with external and internal resources

Schedule appointments with clinicians and/or psychiatric staff

Cover the office/front desk

Maintain the waiting room if children are present

OTHER DUTIES : as assigned by Clinical Director or Program Supervisor at FAC

 
Position Requirements

KNOWLEDGE/SKILLS/ABILITIES:  computer literacy, Fluent Spanish (written and oral)

SUPERVISORY REQUIREMENTS:  no

MINIMUM QUALIFICATIONS:   BA in related field, 2 years related experience

LICENSES/CERTIFICATIONS NEEDED:  N/A
