JOB SUMMARY: This position will receive all new referrals for Mercer County CFS outpatient services.  The main function is to assess the most appropriate referral after pre-intake with client/family.


Determine appropriateness of referral for outpatient

Make referrals as necessary

Regular communication with ACCESS and MRSS staff

Verification of insurance

Phone or in person Pre-Intake

Maintain contact with family until intake takes place

Warm hand off for referral, if internal

If determination is made for external referral, follow up is required

Documentation of all calls

Maintain close working relationships with FGP Program Director and FAC Program Supervisor to learn of all changes to programming, etc

Case Management: including referrals in the community or warm hand off to CHW navigator and/or care coordinator


Work closely with both the CHW Bilingual Navigator and the Crisis response Bilingual Clinician

Possible supervision of graduate students fulfilling field work requirement toward Master’s Degree in Social Work

OTHER DUTIES : as assigned by Clinical Director



Position Requirements

MINIMUM QUALIFICATIONS:     LSW, LAC, LPC, or LCSW and at least 2 years of clinical experience, bilingual a plus



Exempt/Non-Exempt Exempt  
Weekly Work Hours 40 hours  
Schedule Monday-Friday, some evenings required
