Responsible for discharge planning and contributing to the patient’s treatment plan through an understanding of social, psychological, and/or environmental needs related to the impact of illness and need for hospitalization.   Position Requirements MSW required. Experience as a clinical and social worker in a medical setting is preferred. Extensive knowledge of community resources and agencies preferred. Experience with hospital-level discharge planning, including but not limited to rehab, skilled care, or other referrals, and facilitating home care arrangements. Knowledge and understanding of human development and behavior, and the ability to make psychosocial assessments are required.
