Seeking to hire a Full-Time Case Manager

Job summary

  • Offers on-site as well as outreach services
  • Fulfills case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Ensures consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role
  • Communicates observations and information about the consumer in the consumer’s service record and to team members
  • Engages in supportive counseling and crisis intervention
  • Participates in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
  • Collaborates with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
  • Provides input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on regulatory standards; collects information on consumers and reflects all demographic data
  • Participates in quality assurance activities and cross-functional action teams
  • Develop a Plan of Care which should include culturally competent and relevant services and identify the individual goals with measurable objectives the woman wishes to achieve, assess the strengths she has that can be used to work towards those goals, identify barriers that can inhibit goal attainment, and monitor the progress made attaining those goals
  • Ensure the woman engages in services for herself and infant
  • Work collaboratively with the Peer Recovery Specialist to ensure the woman engages in services for herself prenatal, postpartum up to one year
  • Maintain relationships with the woman in order to assist her in the treatment engagement and retention process
  • Reinforce, guide, and ensure the woman that recovery is possible, and is built on multiple strengths, coping abilities, and resources of each individual
  • Empower individuals to make self-determined and self-directed choices about their recovery pathway
  • Provide an initial face-to-face meeting and telephone support; and Maintain follow-up during pregnancy and one year after birth 
 
Position Requirements

Qualifications

  • Bachelor’s degree in a related field
  • Two (2) years experience working within a health care, behavioral health, or substance use treatment setting
  • Substance abuse certification certified preferred, not required– i.e. CADC, LCADC
  • A valid driver's license and must be insurable by agency's automobile insurance
