Seeking to hire a Full - Time Outpatient Coordinator for the CMHC department

Job summary

  • Acts as immediate support to Senior Director, Outpatient Management and Administrative teams
  • Works with staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers and reflects all demographic data 
  • Fulfills case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth 
  • Participates in quality assurance activities and cross-functional action teams 
  • Works with newly hired staff, provides training, and models appropriate professional behavior
  • Coordinates benefit assignments & other entitlements for clients
  • Assists with client enrollment into electronic health record
  • Working with children, families, and adults on their personal treatment goals in order to help create a sense of stability and overall healthy functioning
  • Assesses and evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer
  • Formulates psychosocial treatment plans, with relevant goals, behaviorally measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Provides supportive services and evidence-based interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment
  • Responds to clients for crisis intervention
  • Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated
  • Plans for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
 
Position Requirements

Qualification

  • Master Degree in social work or other behavioral health discipline from an accredited college or university
  • Documented knowledge of one (1) year minimum Clinical work experience with adults and children /adolescent populations
  • Documented knowledge of one (1) year minimum work experience specific to Crisis Intervention
  • Must have provisional license to practice social work/ counseling
  • A valid driver's license and must be insurable by agency's automobile insurance
