About the Organization Easterseals Louisiana (ESL) is a nonprofit organization whose mission is to change the way the world defines and views disabilities by making profound, positive differences in people's lives everyday. Established in 1951, our organization supports over 8,600 individuals (whom we refer to as Participants) with a variety of disabilities. A leading provider of disability services, ESL has physical locations in 8 regions of Louisiana and our array of programs include; support coordination (case management) for children, adults and elderly participants, a wide range of behavioral health and housing services, early intervention services for infants and toddlers, and a school setting called The Chartwell Center in New Orleans.

Mission & Vision:

Job Description:

  • Apply the Housing First approach to serving people with housing and other service needs.
  • Will engage high-risk individuals with a focus on successfully linking them to needed higher level of care.
  • Participates as an active member of Louisiana Balance of State Continuum of Care (BOSCOC).
  • Will conduct necessary assessments, complete mandated legal documentation and complete Individualized Service Plans (ISP) according to funding source and policy timelines.
  • Submit routine reports and outcome data, per contractual requirements, will utilize HMIS system.
  • Attend and participate in weekly staff meetings.
  • Attend and participate in required training.
 
Position Requirements

Qualifications (please take note, we have no flexibility on degrees accepted. These are conditions of our contractor):

  • a Bachelors or Masters in a Human Services related field (i.e. Sociology, Psychology, Education, Counseling, Social Services, Family & Participant Science, Rehabilitation Services, or equivalent.)
  • Experience working in the field of housing services in Baton Rouge (required)
  • Supervisory experience-Preferred
  • A license to practice mental health counseling (e.g. LPC, LCSW)
  • Reliable transportation so that you may make visits to the field to conduct participant visits. You may anticipate about 50% or more of your time will be in the field visits.
  • Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
  • Will be required to pass Easterseals Louisiana background check

Requirements:

  • The employee is regularly required to operate a computer, file and retrieve written documents,
  • communicate with others on the phone and in-person.
  • The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
  • Ability to move independently within the facility and community.
  • Must be able to travel and meet with participants
