The aim of the services is to maintain and assist these older adults to live independently in the community and reduce social isolation. The Case Manager will: evaluate client's presenting problem, determine suitability for the program, and maintain a caseload conduct in-home assessments to identify strengths, goals and resources develop a collaborative helping relationship with client and their family that supports dignity and self-determination of the older adult provide crisis intervention, acquire and provide to clients information and referrals and link clients to socail adult day services, home delivered meals and other supportive services participate in professional development via staff training programs, staff meetings, and supervisory relationship   Position Requirements   This position is currently not accepting applications.
