Share Email Opening Title Licensed Practical Nurse (LPN)   Position Licensed Practical Nurse (LPN)   Full-Time/Part-Time Full-Time   Base Wage $19.90 - $21.87 per hour based on education and experience   Description Summary The LPN is responsible for coordinating and monitoring children’s health and oral health services for the Early Head Start program by advocating for children’s health, promoting optimal health habits, interpreting health records and data to determine appropriate preventive and primary health care for children and families in accordance with Head Start Performance Standards and state regulations.  Essential Duties and Responsibilities include the following. Other duties may be assigned. Collaborate with the Health and Nutrition Manager on the development of health, environmental health and safety, nutrition and emergency policies and procedures and curriculum ensuring compliance with local and state regulations. Provide health education and assistance to parents of children enrolled in the program through various methods. Plan and schedule visits to child care sites, home child care sites, socializations in coordination with family visitors, and home visits to the child’s home as needed. Collaborate and communicate with team members to discuss health requirements based on data reports, documentation, and follow-up. Communicate with medical providers as needed for prescription or other clarifications regarding medical information. Review, evaluate, and interpret health/nutrition/oral health records. Develop and implement individualized health plans (IHPs). Track children with health care needs and those that require medication administration at the EHS sites. Perform all documentation and reporting requirements. Provide training and education to staff, children, and/or families.
