Description

POSITION SUMMARY

Under the direction of the Public Health Director, the Project Manager will administer the CDC Community Health Workers for COVID Response and Resilient Communities grant program. This includes managing budgets, staffing, and reporting; providing technical assistance to partner organizations; and coordinating grant activities across Navajo Nation. Grant activities include pandemic/epidemic preparedness; infectious disease case investigation/contact tracing; vaccination outreach; community health work; community connecting & resource delivery; health education & coaching related to primary, secondary, and tertiary prevention; population health measurement; and public health data support. The incumbent will provide day-to-day coordination for participating 638, IHS, and NN teams. This is a CDC CHW CCR grant-funded position.

ESSENTIAL FUNCTIONS:

  1. Manages the CDC CHW CCR grant budget. Ensures financial requirements of grant are met and reports are submitted.
  2. Drafts MOAs with partner organizations and other contracts. Ensures the adherence to completed MOAs and other contracts.
  3. Completes required grant reporting. Manages evaluation data and performance monitoring.
  4. Provides technical assistance to Public Health Technician teams at all participating organizations in meeting grant activities and ensuring equity in delivery of community health worker services.
  5. Trains and orients public health technicians and other community health workers to contact tracing and other grant activities, roles, and workflows. Ensures competence in all grant activities.
  6. Supervises TCRHCC public health technicians stationed remotely in close collaboration with local supervision.
  7. Travels frequently across Navajo Nation for the purposes of technical assistance, quality assurance, and coordination.
  8. Always maintain professional appearance and personal conduct, representing TCRHCC positively across the Navajo Nation. Ensures that employees stationed remotely do the same.
  9. Ensure compliance to all TCRHCC policies, including employee conduct, HIPAA, and safety procedures
  10. Acts as expert in grant activities, such as public health interventions, case investigation, and community connecting. Obtains training and knowledge to become expert in grant activities.
  11. Provides quality assurance support by observing case, contact, and results notification calls and assessing documentation. Ensures following of evidence-based guidelines.
  12. Provides direction of Public Health Tech Team activities across sites, including the oversight of staff activities related to public health messaging, critical incidents, and epidemiological linkages. Ensures accurate information is reported up chain of command as appropriate.
  13. Updates scripts as needed on results notification, isolation or quarantine, testing, self-care, and emergency signs, symptoms, and recommendations.
  14. Direct staff in connecting community members to available resources, such as food, water, housing, electric, health care services, public health interventions, and social services. Support individuals and families in navigating systems and obtaining culturally appropriate, science-based information.
  15. Supervise staff and ensure competency in providing evidence-based and trauma-informed public health interventions related to mental and physical health promotion, such as the Family Spirit home visiting program. Includes health coaching to support individuals and communities in primary, secondary, and tertiary prevention.
  16. Deliver public health communication and messaging on various prevention topics.
  17. Deliver vaccination efforts and promote public health services to communities, households, and individuals by phone, telehealth technology, or face-to-face in the community.
  18. Collaborate closely with data analyst and epidemiologist to ensure data input and collection.
  19. Provides program evaluation assistance for CDC CHW CCR grant, particularly as it involces obtaining data from other sites.
  20. Assists Community Connectors by providing distribution of food, water and other necessary essentials to community members who are in Isolation or quarantine.
  21. Assists the Contact Tracing team by performing contact tracing activities for patients testing positive for Covid 19, and conducting negative results calling.
  22. Assists the Screener/Greeters at various entry points to ensure appropriate screening for all patients and visitors entering the TCRHCC campus.
  23. Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
  24. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates)
  25. Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.
  26. Performs other duties as assigned.

MENTAL AND PHYSICAL EFFORT

The physical and mental demands described herein are representative of those that must be met by an employee to successfully perform the essential functions of the job.

Physical:

This position requires the incumbent to be able to sit, stand, and walk prolonged periods of time. May also be able to drive, bend, climb, kneel, crouch, twist, maintain balance, and reach. Must be able to lift up to 25pounds. Incumbent must have the ability for hear normal speech, telephone use, far vision, and near vision. Must have the ability for frequent color vision and hearing overhead pages through a loudspeaker. Must also have depth perception and ability for seeing fine details. The position require the ability for hand manipulation with simple and firm grasping and use of keyboards. This position may occasionally be exposed to infectious disease, dust, fumes, gases, and extremes in temperatures or humidity.

Mental:

The work involves assessing and directing community health worker services across Navajo Nation. Must be able to prioritize and use good judgment; coordinate a variety of issues while being frequently interrupted; and be able to cope and manage stressful situations related to the terminal and social impacts issues or concerns among employee, patients and families.Other mental & emotional abilities the Incumbent must have: cope with high level of stress; make decisions under high pressure; concentrate and work alone; handle high degree of flexibility; handle multipe priorities in stressful situation; work alone; demonstrate high degree of patience; and, must be abile to manage altercations by coping with anger/fear/hostility of others in a calm way. Must also be able to accept flexible schedule to meet unit needs.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • Bachelors degree in public health-related field

Experience:

  • One (1) year of experience in managing grants and/or budgets
  • Four (4) years of management, public health, or healthcare experience

Certification:

Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association or obtain within six months from date of hire
