Outstation Outreach Worker (OSOW) 


Job Summary

Assist any patient eligible for services at Yellowhawk Tribal Health Center and their families in applying for alternate resources (i.e. OHP, Medicare B, Medicare D, etc.) and understanding Purchased/Referred Care (PRC) benefits and policies & procedures. All duties and responsibilities will be performed in accordance with Yellowhawk’s mission and vision statement.

Essential Job Functions

  1. Contact with the applicant(s) and initiating the application process.
  2. Assisting the applicants in completing the application for Medicaid or Medicare.
  3. Obtaining required documentation to complete processing of the application.
  4. Ensuring that the information contained on the application form is complete.
  5. Conducting any necessary interviews.
  6. Tracks the application and reapplication process and keeps an updated data base with timelines for meeting renewal application deadlines.
  7. Educates the potential applicants about the potential benefits and requirements of the DMAP programs and Medicare part B & D.
  8. Provides information to individuals about available resource options of medical care through the State of Oregon under the Affordable Care Act.
  9. Provides regular reporting to supervisor regarding interviews, submitted applications, accepted applications, application renewals and other reporting as requested.
  10. Researches the RPMS system for producing priority applicant lists.
  11. Other duties as assigned.
Position Requirements

Minimum Qualifications

  1. High school diploma or equivalent.
  2. Must complete both State and Federal Assistor trainings within the first 60 days of hire.
  3. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  4. Must submit to and pass a reference and criminal background check.
  5. Must submit to and pass a pre-employment drug and alcohol screening.
  6. Must submit proof of COVID-19 vaccination.

Preferred Qualifications

  1. One (1) year of Medical Office experience preferred.

Salary $36,046 - $54,080 annual
