What You Will be Doing
The State Director of Care Coordination & Management provides clinical and operational leadership to the Care Coordination and Care Management Programs throughout the organization. This position is responsible for the planning, budgeting, coordination, and oversight of care coordination and care management services throughout Ohio, overseeing the day-to-day operational and service delivery activities in accordance with local, state, federal, and contractual guidelines and accreditation standards.

  • Provide oversight of all care coordination and care management services delivered, including ensuring engagement targets are achieved and all health or other related outcomes are reported timely as required.
  • Develops and sustains care coordination and care management policies and procedures in collaboration with the team and peers. Participates in other policy development impacting care coordination and care management.
  • Implement a whole person care, integrated care approach to care coordination and care management that prioritizes individuals’ whole health, wellness, and quality of life.
  • Implement evidence and data based standardized care pathways based on health risks and conditions to support individuals served in achieving their best possible health outcomes.
  • Oversee care coordination and care management programs that include the collection, aggregation, analysis, application, and reporting of health, social determinant of health, and delivery of care metrics pertinent to the delivery of care coordination and care management, and/or as required by specific funders or contract.
  • Collaborate with peers within and external to the organization to creatively identify solutions that support optimal health, wellness, and quality of life.
  • Ensure the program staff are complying with all laws, regulations, and accrediting, certifying, or licensing body standards and rules.
  • Provide great customer service within and external to the organization.
  • Identify training and professional development needs within the program and ensure staff have ongoing opportunity for growth, learning, and development—with specific emphasis on evidence and data-based interventions.
  • Serve as a primary point of contact with payers and other external partners in care coordination and participate in stakeholder meetings.
  • Assess needs and advocate for team members to ensure they have the tools necessary to perform their duties successfully.
  • Communicate the vision of program to team members in a way that motivates and inspires excitement, collaboration, and motivation.
  • Support and implement quality and performance improvement projects in collaboration with the Quality Assurance team.
  • Evaluate and interpret business and fiscal data for the purpose of developing new programs and processes to meet business demands.
  • Participate in internal and external committees, workgroups, and task forces.
  • Identify, market, and develop opportunities to expand the delivery of services in collaboration with other Boundless leaders.
  • Complete interviewing, hiring, evaluating, and termination processes as necessary. Ensure performance management and appraisal completion annually per policy for direct reports.

Supervisory Responsibility
This position is directly responsible managing the management or team leadership staff in the care coordination and care management programs. As the program grows and evolves, the titles and number of people directly reporting to this role may change.

Work Environment
This job operates in both professional office environments and in the community where services are delivered. This role routinely uses standard office equipment such as computers, printers, phones, etc.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

Position Type/Expected Hours of Work
This is a full-time position, with remote work flexibility. Hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m, occasional evening and weekend work may be required as job duties demand.

Travel
Travel is anticipated 20-50% of the time within the state of Ohio, primarily during the business day, although some out-of-the-area and overnight travel may be expected on occasion.

 
Position Requirements

Required Qualifications

  • Licensed in the State of Ohio as a Registered Nurse, Independent Social Worker, Psychologist, Professional Clinical Counselor, or Independent Marriage and Family Therapist in good standing.
  • Experience delivering or supervising the activities of care coordination and management.
  • Experience working in the community mental health system in Ohio.
  • Experience serving individuals with complex behavioral health needs, including individuals served by multiple systems such as corrections systems, child welfare, developmental disability services, and others.
  • Five years’ management experience.
  • Five years’ experience clinically supervising or overseeing the delivery of treatment in behavioral healthcare settings.
  • Demonstrated proficiency with personal computers, keyboard and multi-systems navigation, and MS Office Suite applications (Excel, Word, Outlook, PowerPoint, SharePoint, etc.) and prior experience using electronic health record system(s).
  • Ability to travel up 20-50% of the time, mostly in-state. Must have reliable transportation, valid/active driver’s license, and proof of vehicle insurance.
