Job Summary

The Integrated Care Therapist will provide mental health assessment, crisis intervention, safety planning, brief therapy (up to three months), care coordination and other services as part of the Mental Health staff who are integrated in with the primary care teams. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements

Essential Job Functions

  1. Works with the Medical Department, Behavioral Health Department and others to provide crisis intervention, urgent care, risk assessment, mental health screenings, brief therapy, care coordination and warm handoffs.
  2. Completes bio-psycho-social assessment to aid in formulating diagnosis and treatment plan.
  3. Documents delivered services in IHS EHR system and coordinates treatment with the patient, other clinic staff and appropriate community agencies.
  4. Maintains client records, program records and reporting in accordance with Tribal, Indian Health Services, the Federal government, and Oregon State professional standards, rules and regulations. Adheres to all applicable tribal, federal, or state privacy laws and regulations, including but not limited HIPAA and 42 CFR Part 2.
  5. Attends meetings and participates in committees as required by the Mental Health Clinical Manager.
  6. Other duties as assigned.
 
Position Requirements

Minimum Qualifications

  1. Requires a Master’s degree in a behavioral health science field from an accredited university which meets the standards for Oregon licensure for LPC or LCSW.
  2. Candidates not already licensed must actively hold CSWA status and maintain this status while working toward licensure.
  3. Candidates working toward Oregon licensure must provide proof of supervision and be able to obtain licensure within 24 months of hire date.
  4. Demonstrated work/internship experience as a Mental Health Counselor to include experience with suicide risk assessment, crisis intervention, and safety planning.
  5. Minimum of five (5) years active ongoing recovery if recovering from chemical dependency or other addictions.
  6. Ability to work effectively and collaboratively with primary care medical providers, while delivering behavioral health treatment.
  7. Must be detail-oriented, well-organized and able to multi-task while accurately completing activities related to position.
  8. Excellent communication skills both written and orally with the ability to speak before groups.
  9. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
  10. Must submit to and pass a reference and criminal background check.
  11. Must submit to and pass a pre-employment drug and alcohol screening.
  12. Must submit proof of COVID-19 vaccination.

Preferred Qualifications

  1. Previous work experience in delivering behavioral health services in Tribal communities is preferred.
  2. Knowledge of and experience with integrated team based models of care.
  3. Preference will be given to candidates who are licensed as an Oregon Professional Counselor (LPC or LMFT), or an Oregon Clinical Social Worker (LCSW.) Out of state applicants will be required to be accepted for licensure by the State of Oregon during employment introductory probation period.
