Counselor/Case Manager needed for per diem role with mobile support program.
Responsible to serve as a Counselor/Case manager for the organization's Mobile Support Program and provide clinical/case management services to assigned students as per program guidelines through the following: Assess clients' needs as they relate to available services. As applicable, develop and maintain Individual Service Plans (ISP). Provide individual counseling.
Maintain regular contact with parent(s)/guardian(s) regarding clients' progress. Keep accurate case records. As requested, write and submit reports regarding clients' progress. Consult with the Mobile Support Program educational staff weekly and/or as needed regarding clients' progress. Consult as necessary with referral representatives and adjunct professionals regarding clients' treatment and delivery of services. Provide other applicable and essential clinical and case management services. Facilitate program development.
