Case Manager - HWI 


Seeking to hire a full-time Case Manager for the Housing Works Initiative (HWI) department

Job summary

  • Carries designated caseload
  • Assesses clients at intake, during ongoing service delivery and at discharge
  • Collaborates with service providers to establish comprehensive assessments and treatment plans
  • Coordinates continuity of care within the agency, community, jails, hospitals, etc.
  • Formulates comprehensive treatment plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Engages clients in services and addresses their needs; provides or links to health care services, dental care, benefits, mental health services, substance abuse services, family consultation, crisis intervention and vocational/educational/employment services; etc.
  • Recognizes and addresses gaps in service and actively collaborates for resolution
  • Maintains case files on every assigned client
  • Participates in Case Collaboration meetings with supervisor or collaborating agencies, as directed
  • Links clients to community service providers that meet the client's service goals as well as achieve service outcome targets
  • Acts as liaison and maintains open communication with existing case management entities linked with the client. Provides supplemental assistance, as appropriate
  • Communicates client progress, needs and concerns through case review, collaboration meetings and documentation
  • Completes ongoing evaluations/reassessments through regular contact with clients to assess changing needs and progress towards goals. Such contact should consist of home visits, client telephone conferences, client office visits and visits to other community locations
  • Assists clients in completing appropriate forms to ensure their basic financial needs are met (i.e. Medicaid, SSI, SSD, food stamps, GA, etc.)
  • Responds to client-defined crisis, as needed and consults with supervisor regarding each situation before taking action
  • Utilizes own vehicle for outreaches
  • Transports clients as needed in agency vehicle
  • Participates in team building and training, as assigned
  • Participates in special projects, as directed
  • Participates in quality assurance activities and cross-functional action teams
