At Visiting Nurse, we do require our team to be part of the solution to the spread of COVID-19. Week-end Friday-Monday

???????Transition Specialist will communicate with the Physicians, Case Manager, Hospital Discharge Planners and others from the healthcare team to develop an appropriate transition from hospital to home. The Transition Specialist will explain care options to patients and their families and coordinate with healthcare facility staff to ensure that each patient has a smooth transition throughout the discharge process to their home. The Transition Specialist will have a clear understanding of the eligibility requirements for home health care and hospice services. The Transition Specialist utilizes knowledge of Medicare, Medicaid and Commercial insurance to ensure each patient is eligible for services based on payer source requirements.

The Transition Specialist uses the EMR to collect referred patient data onsite and transmit to the agency. The Transition Specialist is also responsible for ensuring the patient has a physician who will oversee the patients' needs at home. The Transition Specialist is also responsible for establishing, growing and maintaining relationships with facility-based referral sources, in accordance with VNHS policies and procedures, by both communicating with existing referral sources and identifying new opportunities.

Position Requirements

Education: Graduate of an accredited Practical Nurse Certificate, Diploma, Associate Degree, Baccalaureate School of Nursing

Licensure/Certifications: Current licensure by the Georgia State Board of Nursing. RN or LPN. Must have current and valid driver’s license.

Previous Experience: 3-5 years medical business experience.

Knowledge and Training: Knowledge of current trends in post-acute care and patient management. Strong multi-tasking abilities, strong communication skills and ability to work well with physicians, patients, hospital staff and co-workers are a must. Knowledge of Medicare/Medicaid and insurance payer patient eligibility requirements.

Skills: Excellent oral and written communication skills to take and write orders. Establish and maintain good working relationships with facility staff and physicians. Ability to identify and respond to VNHS business opportunities within the facility
