Description

Job Summary:

This position will provide primary health care educational and support services to SMI consumers who are enrolled in RIST.

Responsibilities:

Orient RIST consumers to basic principles of primary care management.

  • Consult with RIST consumers and family members to develop and monitor wellness assessments.
  • Assist in care coordination with the project RN to ensure access to primary health care.
  • Help coordinate and ensure consumer access to the array of wellness and recovery education and self-help programs.
  • Participate in regularly scheduled team meetings.
  • Escort consumers to medical appointments and lab appointments, as necessary

 
Position Requirements
  • High School diploma,, proficient in computer skills
  • Possess strong customer service and listening strengths
  • Knowledge of mental illness and ability to act as a good coach for this population
  • Possess excellent written and oral communication strengths and problem solving abilities
  • Completion of the Rutgers Wellness Coaching Training Curriculum
  • Possess ability to function in a team environment
