Responsibilities include and are not limited to:

  • Responsible for the coordination and oversight of the intake process including pre-assessment screening, scheduling of assessments, and short-term case management services to ensure the individual is referred successfully to the appropriate level of care.
  • At initial contact complete pre-screening to evaluate for the appropriate level of care.
  • Utilizes EHR to input new referrals, follow up notes, and ensure proper documentation.
  • Schedules comprehensive clinical assessments.
  • Completes opening paperwork to the agency.
  • Completes crisis plan – ensures the individual is provided a copy along with Crisis Response Numbers.
  • Utilizes case management skills to complete external referrals as needed. Communicates with internal programs to ensure a seamless transition of care for new referrals.
  • Follows up on treatment recommendations to ensure services have been initiated.
Position Requirements


  • Hold Qualified Professional status: Must have a bachelor’s degree from an approved accredited institution; human services preferred.  Spanish speaking is an asset.
  • Experience providing case management.
  • Knowledge of DSM-V diagnoses, provider service options, and community resources required.
  • Demonstrates high moral and ethical character and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry.
  • Possess valid, unrestricted, state-appropriate driver’s license and unrestricted, valid insurance for person vehicles utilized for work.
  • Must be open to working a flexible schedule including evenings and weekends as needed.
  • Must be able to communicate effectively.
  • Must be able to work in a fast-paced environment.
  • Bilingual is a plus!
